The Making a Difference Program (MADP) provides individualised support packages to assist families caring for a family member with a disability in the family home.
The aim is to strengthen the capacities of the families to care for their family member with a disability within the home.
The MADP assists families and carers by providing:
- One-off assistance to families who can usually self manage but require short term assistance
- Case management and individualised funding to families who require assistance to respond to their care needs and maximise their independence and participation in the community.
