Individual Support Packages provide a range of supports to people with a disability, their families, caregivers and communities to better enable them to achieve their goals and aspirations.  Individual Support Packages are funded by the Department of Human Services to assist people to:

  • Live independently in their own home.
  • Remain living in their own home
  • Increase or maintain maximum independence
  • Move to more independent living arrangements
  • Make social connections and participate in community activities 

Support  may include:

  • Direct individual support in the home to maximise independence
  • Access to essential aids and equipment
  • Social and recreational support in the community

Individual Planning

The planner works with the person with a disability and their support network to explore access to informal, formal and specialised disability supports and develops a detailed plan specifically for that person.

Individual Support Packages provide assistance for the development of new skills or help with:

Independent living support such as cooking, menu planning, laundry, managing household finances, decision making, day-to-day problem solving and conflict resolution.

Personal care
- Bathing, hygiene, toileting, grooming, dressing in the person's home.

Community participation
- local community recreation, leisure and cultural activities, using community facilities and services such as banks, shops and libraries and social interaction with others in the community.
